The implementation of the Millennium medical records system in Västra Götalandsregionen has faced severe criticism and delays.
An external review by KPMG revealed significant issues, including an unrealistic timeline, poor preparation, and inadequate training for staff.
The report also highlighted a lack of clear leadership and a tensed working environment, with claims of a "silence culture" under Region Director Håkan Sandahl.
Political leaders, including the Sweden Democrats, have declared their loss of confidence in Sandahl, citing his inability to address the concerns credibly.
The region is now exploring four potential alternatives to the system, but none are expected to be operational for several years.
Meanwhile, the current system used by Närhälsan will expire next summer, adding urgency to the crisis.
